TUITION AND BOOKS: The total tuition and other costs are shown above. Books that are returned to the College at the above noted address for credit may not have any markings and must be undamaged.

CANCELLATION/TERMINATION by the SCHOOL: The College may terminate the enrollment of a student who: 1) fails to pay tuition and fees on time; 2) engages in improper or unlawful conduct; 3) brings discredit to the school; or 4) engages in behavior that interferes with the educational process, or the human or civil rights of another student(s) or staff member(s).

CLASS & COURSE CHANGES: The College reserves the right to cancel any class(s), substitute instructors, and revise or add classes or programs as conditions demand. Classes without a sufficient number of students may be consolidated, canceled, or rescheduled; however, courses required for completion will be offered during the period of time for which the student is enrolled per the Enrollment Agreement. The College reserves the right to make curriculum changes as it deems appropriate. Typically these changes are based on the needs of employers and the recommendations of the programs’ advisory boards, requirements of the Department of Education, the Ohio State Board, and the Accrediting Commission. If the College revises the scheduled start date of any program or cancels any program, applicants for that program have the option of having all fees and prepaid tuition refunded, or the student may elect to attend the program at the revised start date. The maximum possible delay for a start date is 30 class days, after which time the College will refund all fees and prepaid tuition to those students who are not able to begin their program.

GRADUATION REQUIREMENTS: 1) successful completion all program requirements; 2) accumulation of the number of clock hours required for graduation; and 3) satisfaction of all financial obligations.

A passing score on the NABCEP PV Entry Level Exam does not represent any permission or license to work in any field or position. Registration as a NABCEP PV Entry Level Exam Provider does not constitute any endorsement, approval or sponsorship of the Provider or programs by NABCEP, and does not indicate any other affiliation with NABCEP.

GRIEVANCE PROCEDURE: Fortis College is committed to helping each student achieve his or her goals. We are sensitive to the concerns of our students. Any student wishing to express a grievance or complaint should first bring it to the Campus Director for resolution. Issues remaining unresolved at that level may be directed to the Board of Career Colleges and Schools, 35 E. Gay Street, Suite 403, Columbus, OH 43215, .

REFUND AND CANCELLATION POLICIES: If an applicant/student cancels or withdraws or is terminated by Fortis College for any reason, refunds will be made according to Fortis College’s Refund Policy. If a refund is due the student, it will be paid within 30 days of the date that the student either officially withdraws or Fortis College determines that the student has withdrawn. All refunds will be calculated using the student’s last day of class attendance. If a student withdraws without written or verbal notice after classes have started, termination shall be effective on the student’s last date of attendance as determined by the College. Upon receipt of the refund, the student agrees that its receipt constitutes a full and complete release of Fortis College from any and all liabilities.

CANCELLATION / REJECTION POLICY: Fortis College will refund all monies paid by an applicant who is rejected for enrollment by Fortis College, or who enrolls in a program that Fortis College cancels, or who cancels within five calendar days of signing the enrollment agreement.

REFUND POLICY:

1) A student who starts class and withdraws before the program is fifteen per cent completed will be obligated for twenty-five per cent of the tuition and refundable fees plus the registration fee.

2) A student who starts class and withdraws after the program is fifteen per cent complete but before the program is twenty-five per cent completed will be obligated for fifty per cent of the tuition and refundable fees plus the registration fee.

3) A student who starts class and withdraws after the program is twenty-five per cent complete but before the program is forty per cent completed will be obligated for seventy-five per cent of the tuition and refundable fees plus the registration fee.

4) A student who starts class and withdraws after the program is forty per cent completed will not be entitled to a refund of the tuition and fees.

In the event that a student withdraws or is dismissed from the College, all efforts will be made to refund pre-paid amounts for books, fees and supplies except for those items that have been consumed.

If a refund is due, it will be made within 30 days of the withdrawal date, and upon its receipt, the student agrees that its receipt constitutes a full and complete release of Fortis College from any and all liabilities. TRUTH IN LENDING: All program costs are included in this Agreement. Cost of credit is included in the price of the goods and services. There is no carrying, interest, or service charges associated with the student’s program.

CERTIFICATION, REGISTRATION, AND LICENSING: Fortis College does not guarantee that a student who completes his or her program will pass certification, registration, or licensing tests. A student who elects to pursue additional testing is responsible for the costs of those tests. If the student believes he/she can be certified, registered or licensed without attending Fortis College, it is the student’s responsibility to seek out the applicable information. Fortis College is not responsible to provide information on all of the options for certification, registration or licensing.

SEVERABILITY: If any court of competent jurisdiction or governmental or accrediting agency determines that any provision of this Agreement is invalid or unenforceable, such judgment shall not invalidate any other provision of this Agreement.

ARBITRATION: Any disputes or claims arising out of or relating to this Agreement (including any claims against Fortis College, any affiliate of Fortis College or any of the FORTIS affiliate’s officers, directors, trustees, employees, or agents) shall be resolved by individual binding arbitration in accordance with the Commercial Arbitration Rules of the American Arbitration Association then in effect, and judgment on any award by the arbitrator(s) may be entered in any court having jurisdiction. The parties agree that this transaction involves interstate commerce and therefore the Federal Arbitration Act and related federal judicial procedure shall govern this Agreement to the fullest extent possible. The parties agree that any dispute subject to arbitration shall not be adjudicated as a class action or a consolidated class arbitration proceeding either in court or under the rules of the American Arbitration Association. The parties agree that a student’s responsibility to pay administrative fees, filing fees, processing fees, arbitrator compensation, and services charges for arbitration proceedings conducted by the American Arbitration Association under this Agreement shall be limited to no more than $125.00 for claims under $10,000 and $375.00 for claims between $10,000 but less than $75,000, or for claims not seeking monetary compensation. The arbitrator is allowed to ignore this limit, except as prohibited under applicable arbitration rules, should the arbitrator find that the student filed a frivolous claim(s) or unnecessarily delayed the arbitration proceedings. Except as may be required by law, neither a party nor an arbitrator may disclose the existence, content or results of any arbitration conducted pursuant to this provision without the prior written consent of both parties.

STUDENT'S RIGHT TO CANCEL: I understand that I may cancel this agreement and receive a full refund of monies I have paid for tuition and fees, by mailing a written notice to Fortis College postmarked no later than midnight on the fifth calendar day after the date this agreement was signed. I further understand that I may use this page as a cancellation notice by writing "I hereby cancel" at the bottom, and adding my name, date, address and signature, and delivering, or mailing it to Fortis College at the campus address noted at the top of page one of this agreement. I also understand that if I cancel this agreement as noted above more than five calendar days after signing it, and after making an initial payment but prior to the start of classes, I am entitled to a refund of all my payments for tuition and fees.

I agree and acknowledge that I have read this Enrollment Agreement and this agreement constitutes a contract between FORTIS College and me, and that no representations have been made that are not fully expressed herein. Any alterations or revocations of this agreement must be in writing and signed by both FORTIS College and me.
